CONFERENCE REGISTRATION INFORMATION

Enrollment for this conference is limited in order to provide the best possible learning environment. We recommend you register early.

Tuition:

If postmarked by August 8, 2007:
Physicians: $580.00
Nurses, Residents & PA's: $480.00

If postmarked after August 8, 2007:
Physicians: $630.00
Nurses, Residents & PA's: $530.00

Tuition includes continuing education certification, extensive conference syllabus, continental breakfast, group reception, and refreshments.

Tuition may be paid by check, money order, Visa, MasterCard or Discover Card.

Conference registration will be confirmed in writing and more information on travel and hotel will be sent.

Conference cancellations must be confirmed in writing and a 10% of paid tuition processing fee will be assessed. No refunds will be made if cancellation is postmarked after September 12, 2007, however, tuition, less 10% processing fee, may be applied to a future Symposia Medicus conference. In the event of conference cancellation by Symposia Medicus, full tuition refunds will be made.

This conference meets the requirements for CE deductibility under the Tax Reform Act. We suggest you consult your own financial authority for more information.

CONFERENCE LODGING INFORMATION
Symposia Medicus has reserved accommodations at group rates at the beautiful and exciting Atlantis Resort, Paradise Island, Bahamas. Accommodations are available to conference participants and their guests at the rates listed below. These rates will also be effective three days before and three days after the conference, based on availability.

You can qualify for our group rates by using the reservation form in this brochure or by registering on line on our website at www.symposiamedicus.org. Rooms in our block will be released on August 17, 2007. Reserve early as rooms usually sell out before release date.

CONFERENCE GROUP RATES (Single/Double Occupancy):

Coral Tower (Run of House) - $180/night
Coral Tower (Water View) - $265/night

Most Centrally Located Tower
Rooms are approx. 300 sq. ft.
Water View Rooms have Full Balconies

Royal Tower (Run of House) - $210/night
Coral Tower (Water View) - $265/night

Closest to the Spa, Sports Center, Casino
Rooms are approx. 400 sq. ft.
Run of House Rooms have Half Balconies
Water View Rooms have Full Balconies

Rates are per night and do not include Resort Levy of 12% per day, gratuities or other charges, as listed below.

There is a one time charge for porterage of $5.70 per person. Housekeeping gratuities are an additional $11.50 per person per night. Additional gratuities are at your discretion.

Third person (12 years or older) in room is $35.00 extra per night, plus porterage and housekeeping charges.

Children 11 and under are complimentary, using existing bedding, when staying with adults.

Maximum guest room capacity is four adults (ages 12 and over), or two adults and two children under age 12.

Check in time is 3:00pm and check out time is 11:00am.

NOTE: Hotel check-out time is 11:00am. If you check-out after 11:00am, an automatic late charge of $150 will be added to your final hotel bill. If late check-out is needed, please inquire with hotel front desk on the date of departure.

A 30-minute break has been allotted in our conference schedule on Saturday for check-out, if applicable.

Two nights' deposit must accompany the reservation form. The balance of lodging payment, as well as taxes and gratuities, is due to the hotel upon check-in. The hotel accepts all major credit cards.

Hotel cancellations must be made in writing and a full refund of hotel deposit will be made if postmarked by August 15, 2007. Between August 16, 2007, and September 12, 2007, one nights' deposit is forfeited. After September 12, 2007, no hotel refunds can be made. All cancellations must be made in writing and postmarked by the dates stated above.

AIRLINE TRAVEL INFORMATION
You will fly into Nassau International Airport. The airport is approximately 15 miles from Atlantis Resort. You will need to take a taxi to Paradise Island (fare is approximately $30.00), where rental cars are available. Free parking is available at the resort.

Symposia Medicus has arranged group discounts with American and United Airlines for this conference. These airlines may also offer zone fares or area pricing (flat rates with no Saturday night stay requirements). Reservations can be made either through each individual airlines' Convention Desk at the numbers listed below or through your own travel agent.
