CONFERENCE REGISTRATION INFORMATION

• Enrollment for this conference is limited in order to provide the best possible learning environment. Early registration is advised.

• Tuition:

If postmarked by January 6, 2010:
Physicians: $610.00
Residents, PA's & Nurses: $510.00

If postmarked after January 6, 2010:
Physicians: $660.00
Residents, PA's & Nurses: $560.00

• Tuition includes continuing education certification, extensive conference syllabus, group reception and refreshments.

• Tuition may be paid by check, money order, Visa, MasterCard or Discover Card.

• Conference registration will be confirmed in writing and more information on travel, hotel and tours will be sent.

• Conference cancellations must be confirmed in writing and a 10% of paid tuition processing fee will be assessed. No refunds will be made if cancellation is postmarked after February 10, 2010, however, tuition, less 10% processing fee, may be applied to a future Symposia Medicus conference. In the event of conference cancellation by Symposia Medicus, full tuition refunds will be made.

• This conference meets the requirements for CE deductibility under the Tax Reform Act. We suggest you consult with your financial authority for more information.

CONFERENCE LODGING INFORMATION

Symposia Medicus has reserved a variety of accommodations at group rates at the Presidente Inter-Continental Resort in Cancun, Mexico. These rates will also be available three days before and three days after the conference, based on availability, if you would like to extend your stay.

You will receive these group rates by:

• Reserving on-line at www.sympoisamedicus.org.

• Telephone reservations can be made Monday-Friday, 8:30am-5:00pm (Pacific Time), by calling (925) 969-1789 or (800) 327-3161.

• Reservations may also be faxed if paying with Visa, MasterCard, or Discover Card to (925) 969-1795.

• If paying by check, make payable to Symposia Medicus and send with registration form to 399 Taylor Blvd., Suite 201, Pleasant Hill, CA 94523-2300.

Please note that our block of rooms is limited and is on a first-come, first-served basis. PLEASE RESERVE EARLY as rooms may sell out and/or room rates may increase.

All rooms feature cable/satellite television, pay-per-view movie access, nightly turn-down service, coffee maker, mini-bar, in-room safe and iron & ironing board.

• Room rates do not include the following (all subject to change):
- 10% Federal tax
- 2% Lodging tax
- $2.50 housekeeping charge per room, per night
- $4.50 one time, bellman gratuity, per person (13 years and up)

• Maximum room occupancy is three adults OR two adults and two children (12 years and under).

• Group rates INCLUDE daily buffet breakfast for first two people in room.

• Additional occupant charges (includes daily buffet breakfast):
- Children 12 years and under are complimentary in room with adults
- Children 13 to 17 years old are an additional $13 each per night, plus taxes & gratuities
- Third adult (18 & over) is an additional $35 per night, plus taxes & gratuities

• Check-in time is 3:00pm; check-out time is 1:00pm. Late check-out may incur a charge, please verify with front desk upon check-in.

• Cribs and rollaways are complimentary (subject to availability).

• A 30-minute break has been allotted in our conference schedule on Saturday for those needing to check out.

• Two nights' deposit must accompany the reservation form. The balance of lodging payment, as well as any incidental charges, is due to the hotel upon check out. The hotel accepts most major credit cards.

• Per our hotel group contract, hotel cancellations must be made in writing to Symposia Medicus and you will receive a full refund of hotel deposit if postmarked by January 13, 2010. Between January 14, 2010, and February 10, 2010, one night's deposit is forfeited. After February 10, 2010, no hotel refunds can be made. All cancellations must be made in writing and postmarked by the dates stated above.

AIRLINE TRAVEL INFORMATION

You can fly into Cancun International Airport, which is approximately 35 minutes by ground transportation from the hotel. Symposia Medicus has arranged reliable ground transportation from the Cancun airport to the hotel.

Symposia Medicus has arranged group discounts with Alaska, American, Jet Blue and United Airlines for this conference. Reservations can be made either through each individual airlines' Convention Desk at the numbers listed here or through your travel agent.
