CONFERENCE REGISTRATION INFORMATION

• Enrollment for this conference is limited in order to provide the best possible learning environment. Early registration is advised.

If postmarked after February 3, 2010:
Physicians: $660.00

• Tuition includes continuing education certification, extensive conference syllabus, continental breakfasts, group reception and refreshments.

• Tuition may be paid by check, money order, Visa, MasterCard or Discover Card.

• Conference registration will be confirmed in writing and more information on travel, hotel and tours will be sent.

• Conference cancellations must be confirmed in writing and a 10% of paid tuition processing fee will be assessed. No refunds will be made if cancellation is postmarked after March 10, 2010, however, tuition, less 10% processing fee, may be applied to a future Symposia Medicus conference. In the event of conference cancellation by Symposia Medicus, full tuition refunds will be made.

• This conference meets the requirements for CE deductibility. We suggest you consult with your financial authority for more information.

CONFERENCE LODGING INFORMATION

Symposia Medicus has reserved a variety of accommodations at group rates at the beautiful JW Marriott Guanacaste Resort & Spa in Guanacaste, Costa Rica. These rates will also be available three days before and three days after the conference, based on availability, if you would like to extend your stay.

You will receive these group rates by:

• Reservations may also be faxed if paying with Visa, MasterCard, or Discover Card to (925) 969-1795.

Please note that our block of rooms is limited and are on a first-come, first-served basis. PLEASE RESERVE EARLY as rooms may sell out and/or room rates may increase.

All rooms have a balcony or terrace and air conditioning, mini-bar, complimentary in-room coffee/tea service, iron and ironing board, hair dryer, in-room safe, bathrobe, separate tub and shower, iPod port and alarm clock radio, DVD player, cable/satellite color TV, pay-per-view movies/videos, three-line phone, and high speed internet access for a fee.

• Room rates do not include the following (subject to change):

- 13% local taxes

- $6.00 housekeeping charge per room, per night

- $8.00 bellman charge, per person, round trip (13 years and older)

• Children 12 years and under are complimentary in room with adults (note maximum occupancy below).

• Third & fourth person in room (13 years and over) is an additional $35 each, per night, plus taxes.

• Maximum room occupancy is four people.

• Check-in time is 2:00pm; check-out time is Noon.

• Cribs and rollaways are complimentary (subject to availability).

• A 30-minute break has been allotted in our conference schedule on Saturday for those needing to check out.

• Two nights' deposit must accompany the reservation form. The balance of lodging payment, as well as any incidental charges, is due to the hotel upon check out. The hotel accepts most major credit cards.

• Per our hotel group contract, hotel cancellations must be made in writing to Symposia Medicus and you will receive a full refund of hotel deposit if postmarked by February 10, 2010. Between February 11, 2010, and March 10, 2010, one night's deposit is forfeited. After March 10, 2010, no hotel refunds can be made. All cancellations must be made in writing and postmarked by the dates stated above.

AIRLINE TRAVEL INFORMATION

The JW Marriott Guanacaste Resort & Spa is located in Guanacaste, in the north west area of Costa Rica. You can fly into Daniel Oduber Quirós International Airport (formerly known as Liberia International Airport), which is approximately a 1-1/4 hour drive to the hotel; however, lower fares can often be found by flying into Juan Santamaria International Airport, located in Costa Rica's capital city of San Jose, which is approximately a 4-5 hour drive to the hotel. Symposia Medicus has arranged ground transportation from both airports (more information on ground transportation will be sent upon receipt of registration). Additionally, discounts are available through Avis and Budget Rental Car Companies as shown here.
