CONFERENCE REGISTRATION INFORMATION

• Enrollment for this conference is limited in order to provide the best possible learning environment. Early registration is advised.

• Tuition:

If postmarked by February 10, 2010:
Physicians: $610.00
Residents, PAs & Nurses: $510.00

If postmarked after February 10, 2010:
Physicians: $660.00
Residents, PAs & Nurses: $560.00

• Tuition includes continuing education certification, extensive conference syllabus, group reception, and refreshments.

• Tuition may be paid by check, money order, Visa, MasterCard or Discover.

• Conference registration will be confirmed in writing and more information on travel, hotel and activities will be sent.

• Conference cancellations must be confirmed in writing and a 10% of paid tuition processing fee will be assessed. No refunds will be made if cancellation is postmarked after March 17, 2010, however, tuition, less 10% processing fee, may be applied to a future Symposia Medicus conference. In the event of conference cancellation by Symposia Medicus, full tuition refunds will be made.

• This conference meets the requirements for CE deductibility under the Tax Reform Act. We suggest you consult with your financial authority for more information.

Symposia Medicus has reserved a variety of accommodations at group rates at the beautiful Kauai Marriott Resort. These rates will also be available three days before and three days after the conference, based on availability, if you would like to extend your stay.

All rooms are 425 sq. ft. and include a balcony, in-room safe, on-demand movies, complimentary in-room internet access, coffee maker, mini-refrigerator, hair dryer, make-up mirror, and iron/ironing board.

• Room rates stated above are for Single/Double occupancy and do not include the following:

- State & local taxes of 12.42% (subject to change), meals (except daily buffet breakfast)

- Porterage gratuities of $6.25 per person for guests 13 years & older, round-trip

• Maximum room occupancy is four (4) persons, regardless of age.

- Additional person charge includes taxes, gratuities, daily buffet breakfast, and complimentary airport shuttle.

- Children 12 and under are FREE with parents.

- Children 13-18 years are an additional $17 each per night.

- Third and fourth adults, 19 and over, is an additional $65 each per night.

• Rollaways and cribs are complimentary and subject to availability.

• Check-in time is 4:00pm and check-out time is noon.

• A 30-minute break has been allotted in our conference schedule on the last morning of the conference for those who need to check out.

• Two nights' deposit must accompany the lodging reservation form. The balance of lodging payment, as well as any incidental charges, is due to the hotel upon check out. The hotel does not accept personal checks, but major credit cards are accepted.

• Per our hotel group contract, hotel reservation cancellations must be made in writing to Symposia Medicus and a full refund of deposit will be made if cancellation is postmarked on or before February 17, 2010. Between February 18, 2010, and March 17, 2010, one night's deposit is forfeited. If cancellation is postmarked after March 17, 2010, no hotel refunds can be made.

AIRLINE TRAVEL INFORMATION

You will fly into the Lihue Airport on the island of Kauai, just one mile from our conference hotel, the Kauai Marriott Resort.
