• Tuition includes continuing education certification, extensive conference syllabus, continental
breakfast and refreshments.
• Tuition may be paid by check, money order, Visa, MasterCard or Discover.

• If paying by check, make payable to Symposia Medicus, and send with registration form

• Conference registration will be confirmed in writing and more information on travel and hotel
will be sent.
• Conference cancellations must be confirmed in writing and a 10% of paid tuition
processing fee will be assessed. No refunds will be made if cancellation is postmarked
after March 18, 2010, however, tuition, less 10% processing fee, may be applied to a
future Symposia Medicus conference. In the event of conference cancellation by
Symposia Medicus, full tuition refunds will be made.
• This conference meets the requirements for CE deductibility under the Tax Reform Act.

HOTEL RESERVATION INFORMATION
Symposia Medicus has reserved a block of rooms with group rates at the Tuscany Suites & Casino.
The Tuscany Suites & Casino is a self-contained hotel and casino with everything to offer its guests. The hotel, located just a short walk from the center of the World Famous Strip, combines an Italian resort atmosphere with the glamour and excitement of Las Vegas.

Lodging is available to conference participants and their guests at the rates listed below. These
rates will also be effective three days before and three days after the conference if you wish to extend your stay in Las Vegas (based on availability).
Please note that our block of rooms is limited and is on a first-come, first-served basis. PLEASE RESERVE EARLY as rooms may sell out and/or room rates may increase.
GROUP RATE
Guest Room Sunday-Thursday: $65.00/nt (Single/Double Occupancy)
Friday & Saturday: $105.00/nt (Single/Double Occupancy)
All suites feature one king-size or two double beds, a cable television with pay-per-view movie, music, and Nintendo access, coffee maker with complimentary coffee, mini kitchenette with
refrigerator, hairdryer, and deluxe over-sized bathrooms featuring separate soaking tub and shower.
Note:
• A deposit equal to one night's room & tax is due to the hotel with major credit card upon making
your reservation.
• Rates do not include Clark County Room Tax currently 9% (subject to change), meals,
nor gratuities.
• Children 12 years of age and under stay free when sharing accommodations with an adult.
• Third & fourth person in room 13 years and over is an additional $20/person, per night, plus tax.
• Cribs are complimentary & rollways are $20 per night (both subject to availability and prices
subject to change).
• Maximum four guests per room, regardless of age.
• Check in time is 3:00pm and check out time is Noon.
• For early check in, there is a fee of $10 (based on room availability).
• A late check out fee of one-half the daily room rate will be incurred after Noon.
• A 30-minute break has been allotted in our conference schedule on the last day, for those of
you who need to check out of the hotel.
• Hotel cancellations must be made directly with the hotel 48 hours prior to arrival or you will be
charged a cancellation fee equal to your one night's deposit & tax.
