REGISTRATION INFORMATION
• Enrollment for this cruise conference is limited. Early
registration is advised.
• TUITION FEES:
EARLY - If postmarked by May 14, 2010: REGULAR - If postmarked after May 14, 2010: • Tuition includes continuing education certification, extensive
conference syllabus, group reception, and conference refreshments.
• Tuition may be paid by check, money order, Visa, MasterCard,
or Discover Card.
• Register on-line at www.symposiamedicus.org or use the form
in this brochure. If paying by check, make payable and send
with registration form to Symposia Medicus, 399 Taylor Blvd.,
Suite 201, Pleasant Hill, CA 94523-2287; OR
• Telephone registration can be made Monday-Friday,
8:30am-5:00pm (Pacific Time), by calling (800) 327-3161 or
(925) 969-1789. Registrations may also be faxed with Visa,
MasterCard or Discover Card information to (925) 969-1795.
• Conference registration and cruise reservations will be confirmed
in writing and more information on the conference, travel, and the
cruise will be sent to you.
• Conference cancellations must be confirmed in writing and a 10%
of paid tuition fee will be assessed. No tuition refunds will be
made if cancellation is postmarked after June 25, 2010;
however, tuition less 10% processing fee can be applied to a
future Symposia Medicus program. In the event of conference
cancellation by Symposia Medicus, full tuition refunds will be
made. Please see Cruise Cancellation Policy section regarding
cruise cancellations.

CABIN RATES
Rates are PER PERSON, based on double occupancy, and DO NOT include cruise fees of $145 per person, government taxes of $107.15 per person (subject to change), gratuities, possible fuel supplement charge, air transportation, ground transfers, or optional CruiseCare® Cancellation Penalty Waiver Program fees.

Symposia Medicus has contracted for a limited number of cabins for the above categories at the prices listed. If you would like a cabin in a specific category not listed above or the cabin you are requesting is no longer available from our block (sold out), prices above may increase. WE RECOMMEND YOU RESERVE AS SOON AS POSSIBLE.
PLEASE NOTE THE FOLLOWING:
• Total due at booking is $150 per person.
• Final Payment due to Symposia Medicus by Friday, May 14, 2010.
• You will receive your final cruise documents approximately 2 weeks
or more prior to sailing.
• The per person cruise fare for a 3rd and 4th guest in cabin with
two full-fare paying guests is: $604, plus $145 cruise fees,
$107.15 government taxes, and gratuities.
• Single occupancy cabins can be requested, however, the rate for
single occupancy is 200% of the fare for the selected cabin
category.
• Triple and/or quad cabins are limited and not all cabins can
accommodate 3 or 4 people, thus cabin fares quoted above may
increase. Call Symposia Medicus for availability and pricing.
