CONFERENCE REGISTRATION INFORMATION

• Enrollment for this conference is limited in order to provide the best possible learning environment. Early registration is advised.

REGULAR - If postmarked after April 28, 2010:
Physicians: $660
Residents, PA's & Nurses: $560

• Tuition includes continuing education certification, extensive conference syllabus, buffet breakfast, refreshments, and group reception.

• Tuition may be paid by check, money order, Visa, MasterCard, or Discover Card.

• Conference registration will be confirmed in writing and more information on travel, hotel, and activities will be sent.

• Conference cancellations must be made in writing via fax, email, or U.S. mail, and a 10% of paid tuition processing fee will be assessed. No refunds will be made if cancellation is received after June 2, 2010; however, tuition (less 10% processing fee) will be applied toward a future Symposia Medicus conference. In the event of conference cancellation by Symposia Medicus, full tuition and hotel deposit refunds will be made.

• This conference meets the requirements for CE deductibility under the Tax Reform Act. However, we suggest you consult your financial authority for more information.

CONFERENCE LODGING INFORMATION

Symposia Medicus has arranged lodging at group rates at the beautiful Big Sky Resort in Big Sky, Montana. These rates will also be available three days before and three days after the conference, based on availability, if you would like to extend your stay.

You will receive these rates by:

Please note that our block of rooms is limited and rooms are on a first come, first served basis. PLEASE RESERVE EARLY as rooms may sell out and/or room rates may increase.

Big Sky Resort is a full-service resort with several restaurants, heated indoor/outdoor pools, several hot tubs, steam room, sauna, and spa.

Shoshone 1-Bedroom Suite
$243/night (Single/Double Occupancy) (additional people over two, add $10/person per night)

Downstairs of Loft Suite is the same as 1-Bedroom suite. (No Murphy bed). Open spiral staircase goes up to loft. Has a queen or two beds in the loft with a bathroom.

All Summit Rooms are Air-Conditioned. Euro-western luxury hotel adjacent to the Mountain Village. A two-minute walk to the Conference Center.

All Village Rooms are Air-Conditioned. Big Sky Resort's newest section in the heart of the Mountain Village. Located between the Summit and Shoshone buildings.

*Summit and Village units that are VERY limited. Please reserve early.

• Maximum occupancy in rooms cannot be exceeded.

• The Big Sky Resort is a smoke-free environment.

• Rates DO NOT include 7% state tax, 3% resort tax, 7% resort service fee, meals (unless otherwise stated), nor gratuities (all subject to change without notice)

• Children 10 and under stay free in room with parents with existing bedding (not to exceed maximum occupancy).

• Two nights' room deposit must accompany the lodging reservation form. The balance of lodging payment, as well as any incidental charges, is due to the hotel upon check out.

• Check-in time for all units is 5pm. Check-out time is 10am for the Shoshone, Summit & Village buildings, and noon for the Huntley Lodge.

• Per our hotel contract, hotel cancellations must be made in writing to Symposia Medicus and you will receive a full refund if postmarked by May 5, 2010. Between May 6, 2010, and June 2, 2010, one nights' deposit is forfeited. After June 2, 2010, no hotel refunds can be made. All cancellations must be made in writing and postmarked by the dates stated above.

AIR TRAVEL INFORMATION

You will be flying into Bozeman, Montana, which is approximately 1 hour from the resort.

Symposia Medicus has arranged for discounts with Alaska and United Airlines for this conference. Reservations can be made either through each individual airlines' Convention Desk at the numbers listed below or through your travel agent.
