CONFERENCE
REGISTRATION INFORMATION Enrollment for this conference is limited in order to provide the best
possible learning environment. Early registration is advised.
TUITION FEES:
EARLY REGULAR Tuition includes continuing education certification, extensive conference syllabus, continental
breakfast, refreshments, and wine & cheese reception.
Tuition may be paid by check, money order, Visa, MasterCard, or Discover Card.

Conference registration will be confirmed in writing and more information on travel, hotel, and
group tours will be sent.
Conference cancellations must be confirmed in writing and a 10% of paid tuition processing
fee will be assessed. No refunds will be made if cancellation is postmarked after May 19, 2010;
however, tuition less 10% processing fee, may be applied to a future Symposia Medicus
conference. In the event of conference cancellation by Symposia Medicus, full tuition
refunds will be made.
This conference meets the requirements for CE deductibility under the Tax Reform Act. However,
we suggest you consult your financial authority for more information.

CONFERENCE LODGING INFORMATION
Symposia Medicus has reserved accommodations with group rates at the Napa Valley Marriott Hotel
& Spa. Lodging is available to registrants and their guests at the rates listed below. These rates will also be available three days before and three days after the conference (based on availability) if you wish to extend your stay.

Please note that our block of rooms is limited and is on a first come, first served basis. PLEASE RESERVE EARLY as rooms may sell out and/or room rates may increase.
GROUP RATES - Single/Double Occupancy
Standard Room Concierge Level All rooms are approx. 375 sq. feet and include pillowtop mattresses, mini refrigerators, coffeemakers with complimentary coffee, wireless internet (for a fee), pay-per-view movie access and flat screen TVs. All rooms have either Garden or Street views.
Rates above are per room, per night, and do not include tax of 12%, nor California Tourism
Assessment Fee of 0.065% (both subject to change), meals, nor gratuities, unless otherwise stated.
Maximum occupancy is 4 people and cannot be exceeded.
There is no additional charge, for our group, for 3rd and 4th adult (18 years and older) nor children
sharing a room with an adult and using existing bedding.
Cribs and roll-away beds are complimentary (subject to change).
Two nights' room deposit must accompany the lodging reservation form. The balance of lodging
payment, as well as any incidental charges, is due to the hotel upon check out.
Check-in time is 4:00 pm and check-out time is Noon. (Requests for late check-out need to be
made with hotel front desk on day of departure).
A 30-minute break has been allotted in our conference schedule on the last day for those of you
who need to check out.
Per our hotel contract, hotel cancellations must be made in writing to Symposia Medicus and
you will receive a full refund if postmarked by April 21, 2010. Between April 22, 2010, and
May 19, 2010, one night's deposit is forfeited. After May 19, 2010, no hotel refunds can be
made. All cancellations must be made in writing and postmarked by the dates stated above.
AIRLINE TRAVEL INFORMATION
The closest airport to the Napa Valley Marriott Hotel & Spa is Oakland International Airport; however, lower fares can sometimes be found by flying into San Francisco International Airport, Sacramento International Airport or San Jose International Airport. Oakland is approximately 50 miles from the hotel, while San Francisco and Sacramento are approximately 60 miles and San Jose is
approximately 85 miles.
Symposia Medicus has arranged group discounts with Alaska, American, Jet Blue, and United Airlines for this conference. Reservations can be made either through your travel agency or through the airlines' Convention Desk at the numbers listed below.
