• Enrollment for this conference is limited in order to provide the best
possible learning environment. Early registration is advised.
• TUITION:
EARLY REGULAR • Tuition includes continuing education certification, extensive conference syllabus, refreshments, and a group reception.
• Tuition may be paid by check, money order, Visa, MasterCard, or Discover Card.

• Conference registration will be confirmed in writing and more information on travel, hotel, and activities will be sent.
• Conference cancellations must be made in writing via fax, email, or U.S. mail, and a 10% of paid tuition
processing fee will be assessed. No refunds will be made if cancellation is postmarked after June 23, 2010;
however, tuition (less 10% processing fee) will be applied toward a future Symposia Medicus conference.
In the event of conference cancellation by Symposia Medicus, full tuition and hotel deposit refunds will be made.
• This conference meets the requirements for CE deductibility under the Tax Reform Act. However, we suggest you consult
your financial authority for more information.

CONFERENCE LODGING INFORMATION
Symposia Medicus has reserved a variety of accommodations with group rates at the Snow King Resort & Grand View Lodge. Lodging is available to registrants and their guests at the rates listed below. These rates will also be available three days before and three days after the conference (based on availability) if you wish to extend your stay.
You will receive these rates by:

Please note that our block of rooms is limited and is on a first-come, first-served basis. PLEASE RESERVE EARLY

(Rooms are very
limited, please reserve early) Resort fee, included in room rate, includes: Complimentary Wi-Fi, special guest pricing & discounts on
Snow King recreational activities, on-property transportation (to/from condos), complimentary parking and
access to sauna/exercise room & game room.
• Rates above are per room, per night, and do not include hotel & state tax (currently 11%) and subject to change, meals
(unless otherwise stated) nor gratuities.
• Additional occupants (14 yrs & older) are $10 per person, per night (not to exceed maximum occupancy).
• Children 13 years and under are complimentary, using existing bedding, when staying with adults.
• Maximum occupancy stated cannot be exceeded.
• Cribs are complimentary and rollaways are $10 per night (subject to change).
• Two nights' room deposit, on room rate only, must accompany the lodging reservation form. The balance of lodging
payment, as well as any incidental charges is due to the hotel upon check out.
• Check-in time is 3:00 pm and check-out time is Noon. (Late check-out can only be confirmed on the date of
departure by the front desk. Additional charges will apply for late check-outs and are based upon availability).
• A 30-minute break has been allotted in our conference schedule on the last day for those of you who need to
check out.
• Per our hotel contract, hotel cancellations must be made in writing to Symposia Medicus and you will receive a full
refund if postmarked by May 26, 2010. Between May 27, 2010, and June 23, 2010, one night's deposit is forfeited.
After June 23, 2010, no hotel refunds can be made. All cancellations must be made in writing and postmarked by
the dates stated above.
AIR TRAVEL INFORMATION
You will be flying into Jackson, Wyoming, which is approximately 20 minutes from the resort. Symposia Medicus has arranged
for discounts with Alaska, American and United Airlines for this conference. Reservations can be made either through each
individual airlines' Convention Desk at the numbers listed below or through your travel agent.
