• Tuition includes continuing education certification, extensive conference syllabus, continental breakfast Wednesday, Thursday and Saturday, refreshments and group reception.

• Conference registration will be confirmed in writing and more information on travel, hotel and group tours will be sent.

• Conference cancellations must be confirmed in writing and a 10% of paid tuition processing fee will be assessed. No refunds will be made if cancellation is postmarked after August 11, 2010, however, tuition less 10% processing fee, may be applied to a future Symposia Medicus conference. In the event of conference cancellation by Symposia Medicus, full tuition refunds will be made.

• This conference meets the requirements for CE deductibility under the Tax Reform Act. We suggest you consult with your financial authority for more information.

CONFERENCE LODGING INFORMATION
Symposia Medicus has arranged lodging at group rates at the beautiful Inn & Spa at Loretto. These rates will also be available three days before and three days after the conference, based on room availability, if you would like to extend your stay. Inn & Spa at Loretto is a full-service resort with casual fine dining, a full-service spa, an outdoor pool, three art galleries, and a tranquil garden.

Two nights' room deposit must accompany the lodging reservation form. The balance of lodging payment, as well as any incidental charges, is due to the hotel upon checkout.

Please note that our block of rooms is limited and are on a first-come, first-served basis. PLEASE RESERVE EARLY as rooms may sell out and/or room rates may increase.

Traditional rooms have large picture windows that open out. Superior & Deluxe rooms have balconies. All rooms have pillow-top mattresses and feather comforters, wireless internet access, flat-screen television with cable & pay-per-view movie channels, fully stocked mini-bar, in-room coffee and tea, iPod docking station, hair dryer, iron and ironing board. All rooms have views of the Sangre de Cristo Mountains, Saint Francis Cathedral or Loretto Chapel.

• Rates are per room, per night, and do not include tax (currently 14.94% - subject to change).

• Rates also do not include a mandatory charge of $2.00 per room, per night which covers housekeeping gratuities, high-speed wireless internet access, and local and toll free telephone calls.

• Children (18 & under) are free in room with adults.

• 3rd & 4th Adult in room (19 & over) is $30.00 each per night.

• Maximum occupancy in room is four (regardless of age).

• Cribs and rollaways are complimentary (subject to availability)

• Check-in time is 4:00pm and checkout time is 11:00am. For a late check out, inquire with the front desk on day of departure. Additional charges may apply.

• To avoid an "early departure charge," you must advise the hotel at or before check-in of any changes in the scheduled length of stay.

• "No shows" will be charged at the nightly rate for the entire reserved stay.

• Per our hotel contract, hotel cancellations must be made in writing to Symposia Medicus and you will receive a full refund if cancellation is postmarked by July 14, 2010. Between July 15, 2010, and August 11, 2010, one night's deposit is forfeited. After August 11, 2010, no hotel deposit refunds can be made. All cancellations must be made in writing and postmarked by the dates stated above.

AIR TRAVEL INFORMATION
We recommend you fly into Albuquerque International Sunport Airport (68 mile drive to Santa Fe) as flights in/out of Santa Fe Municipal Airport (9 miles from the hotel) are usually more expensive.

Symposia Medicus has arranged group discounts with American and United Airlines for this conference. Reservations can be made either through each individual airlines' Convention Desk at the numbers listed here or through your travel agent.
