• Tuition Fees:
EARLY REGULAR • Tuition includes continuing education certification, extensive conference syllabus,
continental breakfast and refreshments.
• Tuition may be paid by check, money order, Visa, MasterCard or Discover.

• If paying by check, make payable to Symposia Medicus, and send with registration

• Conference registration will be confirmed in writing and more information on travel and
hotel will be sent.
• Conference cancellations must be confirmed in writing and a 10% of paid tuition
processing fee will be assessed. No refunds will be made if cancellation is
postmarked after August 16, 2010, however, tuition, less 10% processing fee, may
be applied to a future Symposia Medicus conference. In the event of conference
cancellation by Symposia Medicus, full tuition refunds will be made.
• This conference meets the requirements for CE deductibility under the Tax Reform Act.
However, we suggest you consult your financial authority for specific interpretation.

CONFERENCE LODGING INFORMATION Symposia Medicus has reserved a block of rooms at group rates listed below at the Flamingo Las Vegas Hotel & Casino. The hotel, located right on the famous Las Vegas Strip,
combines a resort atmosphere with the glamour and excitement of Las Vegas.
Lodging is available to conference participants and their guests at the rates listed below. These rates will also be effective three days before and three days after the conference (based on availability) if you wish to extend your stay.
Please note that our block of rooms is limited and is on a first come, first served basis. PLEASE RESERVE EARLY as rooms may sell out and/or room rates may increase.
GROUP RATES:
Deluxe Room Go Deluxe Room • Rates do not include tax currently 9% (subject to change), meals, nor gratuities.
• One night's room deposit, including tax, is due at time of reservation.
• Each guestroom must have at least one registered guest 21 years of age or older.
• Children 18 years of age and under stay free when sharing accommodations with an adult.
• Third & fourth adult 19 years of age or older is $30.00 per person, per night.
• Maximum four guests per room.
• Rollaway beds are an additional $30 per room, per night (subject to availability).
• Cribs are complimentary (subject to availability).
• Check-in time is 4:00pm and check-out time is 11:00am. (Departures after 11:00am
are subject to a full day charge)
• A longer break has been allotted in our conference schedule on Wednesday, for those
of you who need to check out of the hotel.
• To avoid an early check-out fee of $50 plus tax, you should advise the hotel at
or before check-in of any change in your planned length of stay.
• Hotel cancellations must be made directly with the hotel 72 hours prior to scheduled
arrival or you will be charged a cancellation fee equal to your one night's deposit & tax.
