REGISTRATION INFORMATION
• Enrollment for this conference is limited in order to provide the best
possible learning environment. Early registration is advised.
• TUITION:
EARLY REGULAR • Tuition includes continuing education certification, continental breakfast, and refreshments.
• Tuition may be paid by check, money order, Visa, MasterCard or Discover.

• If paying by check, make payable to Symposia Medicus, and send with registration
form to 399 Taylor Blvd., Suite 201, Pleasant Hill, CA 94523-2287; OR
• Telephone registrations can be made Monday-Friday, 8:30am-5:00pm (Pacific Time),
by calling (925) 969-1789 or (800) 327-3161. Registrations may also be faxed if
paying with Visa, MasterCard or Discover Card to (925) 969-1795.
• Conference registration will be confirmed in writing and more information on travel and
hotel will be sent.
• Conference cancellations must be confirmed in writing and a 10% of paid tuition
processing fee will be assessed. No refunds will be made if cancellation is
postmarked after August 26, 2010, however, tuition, less 10% processing fee, may
be applied to a future Symposia Medicus conference. In the event of conference
cancellation by Symposia Medicus, full tuition refunds will be made.
• This conference meets the requirements for CE deductibility under the Tax Reform Act.

CONFERENCE LODGING INFORMATION Symposia Medicus has reserved a block of rooms at the group rates listed below at the
historic Fairmont Hotel, in San Francisco, California.
Lodging is available for conference participants and their guests at the rates listed below. These rates will also be effective three days before and three days after the conference (based on availability) if you wish to extend your stay.
Please note that our block of rooms is limited and is on a first come, first served basis. PLEASE RESERVE EARLY as rooms may sell out and/or room rates may increase.
Run of House Tower Deluxe Tower Signature All rooms feature a large picture window, and have one king or two queen beds, Frette
linen ensemble with pillow top beds, flat screen TV's, and MP3 docking stations.
Tower Deluxe and Tower Signature rooms include oversized marble baths with
separate shower & tub, and a large desk in a sitting area.
• Rates do not include occupancy tax of 14%, and a Tourism Improvement District
Assessment of 1.5% (subject to change without notice), meals nor gratuities.
• Maximum persons in room are 4.
• Third & fourth adult 19 years of age or older is $30.00 per person, per night, plus tax.
• Children 18 & under stay free in room with parents.
• Check-in time is 3:00pm and check-out time is Noon (late check-outs need to be
confirmed with the front desk at check-in, and a late check-out charge may apply).
• To avoid an early departure fee of $50, changes must be made to your confirmed
departure date no later than at time of hotel check-in.
• A 30-minute break has been allotted in our conference schedule on the last day, for those
of you who need to check out.
• The Fairmont Hotel offers parking for overnight guests at $50/day plus prevailing taxes
(currently 14%), with in and out privileges. The guest parking rate is subject to change
without notice and is based on availability. Information on possible lower parking rates
at alternate parking garages will be sent with your Confirmation Letter.
• Hotel cancellations must be made directly with the hotel 72 hours prior to confirmed
check-in date or a cancellation fee equal to one night's deposit will be charged.
AIRLINE DISCOUNTS/INFORMATION Symposia Medicus has arranged for discount fares with Alaska, American, Jet Blue, and United Airlines for this conference. Reservations can be made either through your own travel agency or through the airlines Convention Desk at the numbers listed below.
In order to receive the discounted rate, you or your travel agent must refer to the Symposia Medicus Group ID number listed below.
