CONFERENCE REGISTRATION INFORMATION

Enrollment for this conference is limited in order to provide the best possible learning environment. Early registration is advised.

Tuition includes continuing education certification, extensive conference syllabus, group reception, and refreshments.

Tuition may be paid by check, money order, Visa, MasterCard or Discover.

Conference registration will be confirmed in writing and more information on travel, hotel and activities will be sent.

Conference cancellations must be confirmed in writing and a 10% of paid tuition processing fee will be assessed. No refunds will be made if cancellation is postmarked after September 15, 2010, however, tuition, less 10% processing fee, may be applied to a future Symposia Medicus conference. In the event of conference cancellation by Symposia Medicus, full tuition refunds will be made.

This conference meets the requirements for CE deductibility under the Tax Reform Act. We suggest you consult with your financial authority for more information.

CONFERENCE LODGING INFORMATION
Symposia Medicus has reserved a variety of accommodations with group rates at the Marriott Frenchman's Reef & Morning Star Beach Resort. Lodging is available for registrants and their guests at the rates listed below. These rates will also be available three days before and three days after the conference (based on availability) if you wish to extend your stay.

Two nights' deposit must accompany the reservation form. The balance of lodging payment, as well as any incidental charges, is due to the hotel upon check out. The hotel accepts most major credit cards.

Please note that our block of rooms is limited and is on a first come, first served basis. PLEASE RESERVE EARLY as rooms may sell out and/or room rates may increase.

GROUP RATES (Single/Double Occupancy):

Resort View: $160/night
Rooms are located in the main building of hotel, with views of island gardens & hills. No balcony.

Water View: $191/night
Rooms are located in the main building of hotel, with a water view. Rooms have a 40 sq.ft. balcony.

*Morning Star Garden View: $230/night
Located on the beach. Rooms overlook lush tropical gardens from a 40 sq. ft. balcony.

*Morning Star Beachfront View: $260/night
Located on the beach. Rooms have a view of the Caribbean Sea from a 40 sq. ft. balcony.

All rooms are 400 sq. ft. and have one king bed or two double beds, air-conditioning, shower/tub combination, hair dryer, safe, wireless internet for a fee, 19 inch LCD TV with premium movie channels, iron/ironing board, coffee maker/tea service, and mini-refrigerator.

*Morning Star rooms are limited. Please reserve early.

Room rates do not include the following (subject to change):

8% state & local taxes per room, per night.

5% porterage & housekeeping gratuities per room, per night (additional gratuities are at your discretion).

Maximum room occupancy is four.

Children 12 years and under are complimentary, using existing bedding, when staying with adults.

Third & fourth person(s) in room (13 years & over) is an additional $40 each, plus tax, per night.

Check-in time is 4:00pm; check-out time is 11:00am.

Cribs and rollaways are complimentary (subject to availability).

A 30-minute break is in our conference schedule on Saturday for check-out, if applicable.

A $50.00 fee will be charged by the hotel for unscheduled early departures. You must notify the hotel at check-in of any change in your scheduled departure to avoid this fee.

Per our hotel group contract, hotel cancellations must be made in writing to Symposia Medicus and you will receive a full refund of hotel deposit if postmarked by August 18, 2010. Between August 19, 2010, and September 15, 2010, one night's deposit is forfeited. After September 15, 2010, no hotel refunds can be made. All cancellations must be made in writing and postmarked by the dates stated above.

AIRLINE TRAVEL INFORMATION
You will fly into Cyril E. King Airport, (STT), in St. Thomas. The airport is approximately 6 miles from the Marriott Frenchman's Reef & Morning Star Beach Resort.

Symposia Medicus has arranged group discounts with American and United Airlines for this program. Reservations can be made either through each individual airlines' Convention Desk at the numbers listed below or through your own travel agent.
