CONFERENCE REGISTRATION INFORMATION Enrollment for this conference is limited in order to provide the best possible learning

TUITION:
EARLY REGULAR Tuition includes continuing education certification, extensive conference syllabus, continental
breakfast (Wednesday, Thursday, and Saturday), refreshments, and a group reception.
Tuition may be paid by check, money order, Visa, MasterCard, or Discover Card.

Conference registration will be confirmed in writing and more information on travel, hotel,
and tours will be sent.
Conference cancellations must be made in writing via fax, email, or U.S. mail, and a
10% of paid tuition processing fee will be assessed. No refunds will be made if
cancellation is postmarked after September 8, 2010; however, tuition (less 10%
processing fee) will be applied toward a future Symposia Medicus conference. In the
event of conference cancellation by Symposia Medicus, full tuition and hotel deposit
refunds will be made.
This conference meets the requirements for CE deductibility under the Tax Reform Act.
However, we suggest you consult your financial authority for more information.

CONFERENCE LODGING INFORMATION
GROUP RATES have been arranged at the beautiful Atlantis Resort on Paradise Island, Bahamas. These rates will also be available three days before and three days after the
conference, based on availability, if you would like to extend your stay.
Two nights' deposit must accompany the reservation form. The balance of lodging
payment, as well as resort levies, resort fees & gratuities, is due to the hotel upon
check-out. The hotel accepts all major credit cards.

GROUP RATES (Single/Double Occupancy):
Royal Tower
Run of House: $238/night (Single/Double Occupancy) Royal Tower Water View: $328/night (Single/Double Occupancy) Royal Tower is at the center of
activities & entertainment including the casino, Mandara Spa, Aura
nightclub, Dolphin Cay, AquaVenture and several dining options. Rooms
are approx. 400 sq ft, with
step out French balcony.
ROOM AMENITIES Rates are per night and do not include the following fees (subject to change):
Taxes and resort levies per room per day (on room rate only): 6% guest room tax,
3% tourism levy (billed as PITDA), and 3% promotion board levy (billed as NPIPB)
One time bellman gratuity charge of $6.00 per person (age 12 and older)
Housekeeping gratuity charge of $5.00 per person, per night (age 12 and older)
Utility service fee (government tax for electricity) of $7.50 per person, per day
(age 12 and older)
Pool/beach attendant/lifeguard fee of $1.60 per person, per day (age 12 and older)
Additional gratuities are at your discretion.
Maximum guest room capacity is four adults (age 12 and older), or two adults & two
children under 12 years.
Children 11 & under are complimentary, using existing bedding, when staying with adults.
Third & fourth person (age 12 and older) in room are $50.00 extra per night, plus fees as
stated above.
Cribs & roll-away beds are complimentary (subject to availability).
Check-in time is 3:00pm and check-out time is 11:00am. (NOTE: If you check-out after
11:00am, an automatic late charge of $150 will be added to your final hotel bill. If late
check-out is needed, please inquire with hotel front desk on the date of departure.)
Per our hotel group contract, hotel cancellations must be made in writing to
Symposia Medicus and you will receive a full refund of hotel deposit if postmarked
by August 11, 2010. Between August 12, 2010, and September 8, 2010, one night's
deposit is forfeited. After September 8, 2010, no hotel refunds can be made. All
cancellations must be made in writing and postmarked by the dates stated above.
AIRLINE TRAVEL INFORMATION
You will fly into Nassau International Airport. The airport is approximately 15 miles from the Atlantis Resort on Paradise Island. You can take a taxi to the resort (fare is approximately $30), or rental cars are available. The resort has free self-parking.
Symposia Medicus has arranged group discounts with American and Jet Blue Airlines for this conference. Reservations can be made either through each individual
airlines' Convention Desk at the numbers listed below or through your travel agent.
